When should you book a surgical consultation?

A consultation with an endocrine surgeon is recommended in the following situations. A referral from your GP is not mandatory to book.

Thyroid nodule Particularly when biopsy results are indeterminate or suspicious (Bethesda III–VI), when the nodule is growing, or when it causes compressive symptoms (difficulty swallowing, breathing discomfort).

Goitre Causing difficulty swallowing, breathing restriction or a sensation of pressure in the neck.

Hyperthyroidism Graves' disease or toxic autonomous nodule, when medical treatment or radioiodine is not suitable or has failed.

Thyroid cancer Confirmed or suspected on cytology, for surgical planning and discussion of extent of resection.
🔷
Primary hyperparathyroidism Elevated calcium and PTH, to assess the surgical indication, confirm gland localisation and discuss whether parathyroidectomy is appropriate.

The surgical consultation typically lasts 30 to 45 minutes. There is no obligation to proceed with surgery following the consultation — the decision is made jointly, with full information, and at your own pace.

Dr. Guian practises as a Sector 2 surgeon with OPTAM-CO membership, which caps additional fees and enables significantly better reimbursement from complementary health insurance (mutuelle) compared to a standard Sector 2 practitioner. The Carte Vitale is accepted.

Do I need a referral to book an appointment?

No referral is required to book a surgical consultation. However, a referral from your GP (médecin traitant) under the coordinated care pathway improves your reimbursement rate from national health insurance. If you have a referral, please bring it to the appointment or upload it when booking on Doctolib.

How long is the wait for an appointment?

The typical waiting time for a first consultation is under two weeks. You can check real-time availability directly on Doctolib. For urgent situations, please call the practice at 01 45 02 18 18 — we will do our best to accommodate you as quickly as possible.

Can the consultation be done remotely (telemedicine)?

Thyroid and parathyroid surgical consultations require a clinical examination and in-person review of imaging. An initial teleconsultation is not appropriate for a first surgical assessment. A follow-up teleconsultation after surgery may be possible in selected situations — please ask at the practice.

Is the consultation available in English?

Yes. Dr. Guian conducts consultations in English for international or non-French-speaking patients. Please mention your language preference when booking so the appropriate slot can be reserved.

What happens if I decide not to have surgery after the consultation?

There is no obligation to proceed. If surgery is not indicated, or if you prefer to defer the decision, Dr. Guian will advise on the appropriate surveillance protocol and can write to your GP or referring specialist with a summary of the consultation findings.
